| 2 Herniated discs of the cervical spine | Which possible methods of treatment do you see from the following MRI scan results: vertical position of the cervical spine in the cranial segment with a bend at the level of the C5/6. Normal height and normal signal reaction of the vertebral body. In the middle and lower cervical spine slight bony extensions on the edge of the
end bone plates and degenerative signal-/ height reduction of the inter-vertebral space with concentric disc protrusions. In the segments C 4/5 and C5/C6 wide basal deposits of disc tissue can be seen especially in the spinal canal of C5/6. Restriction of the spinal canal in the segments C4/5. Restriction of the neuroforamina of the C3/4 on the left, C4/5 on the left, C5/6 on both sides and C6/7 on the right. Normal bone marrow. Regular craniocervical transition.
Assessment: Vertical position and osteochondrosis of the cervical spine in particular the segments C4/5 to C6/7.
Wide basal herniated disc in the segment of the C4/5, on the left mediolateral herniated disc in the segment C5/6.
Maximal spinal canal restriction with bone marrow compression in the segment C5/6, less in C4/5.Restriction of the neuroforamina in the segments C3/4 on the left, C4/5 on the left, C5/6 on both sides and C6/7 on the right.
Contrary to all expectations I am not in pain, however I do have tingling to numb feelings in the right hand (increasing):Considerable limitation of the fine motoric and some coordination problems when walking.
I am supposed to have an operation. The discs should be removed and replaced with bone fragments from the pelvic girdle. I did not however want a fusion of the cervical spine. Do you see any other possibilities?

| Dear Mrs H.,
I would recommend physical therapy as the first measure. If this did not lead to prolonged improvement of the condition, a Prof. Racz epidural spinal catheter could be considered to reduce the compromised structures in the spinal canal. The operation you describe would be in my opinion the last way to treat therapy resistant conditions.
